I got the long "D" bolts too. But I bought some shorter 12mm cap screws fully threaded and used a grinder to make the "D" shape on the head. :D

If GFCI's are "dead" usually they don't have power or are bad (GFCI's failing is pretty rare). Or possibly hooked up wrong. Power to the GFCI needs to be connected to the line side of the unit.
