I've seen it happen on a riding buddy's bike. You have to be careful when installing the tube so that doesn't happen. It helps if you inflate the tube a bit before you stuff it into the tire.

The crush washer was pretty worn. If this doesn't fix it, I'll dig deeper. Thanks for the suggestions.
